Please fill in this form if you are doing a course with us. If you tick yes to any question you must have your GP or a diving doctor sign you as fit to dive. If you answer yes we cannot teach you any course without a doctor's signoff.

If your GP will not complete the form, please contact the Diving Diseases Research Centre on 01752 228825 (they will charge for each medical).
